DÉCHARGE DE RESPONSABILITÉ  

• Je, soussigné(e), participant(e), reconnais et comprends les risques associés à la participation aux activités d'arts martiaux proposées par The Kliq MMA INC (ci-après dénommé "The Kliq"). Je participe volontairement à ces activités et je suis conscient(e) qu'elles impliquent des contacts physiques et un risque de blessure.
• Je certifie être en bonne condition physique et ne pas avoir de conditions médicales m'empêchant de participer aux activités d'arts martiaux. Si j'ai des conditions médicales, des allergies ou des blessures, j'ai fourni des informations exactes ci-dessus et je m'engage à informer The Kliq de tout changement concernant mon état de santé.
• Je comprends et accepte que l'entraînement aux arts martiaux comporte des risques inhérents, notamment des tensions musculaires, des entorses, des fractures, traumatisme cérébrale et d'autres blessures potentielles. J'accepte que The Kliq et ses instructeurs prennent des précautions raisonnables pour assurer ma sécurité, mais des accidents peuvent néanmoins survenir.
• Par la présente, je dégage The Kliq, ses propriétaires, instructeurs, employés et bénévoles de toute responsabilité, réclamation, demande ou action en justice découlant de toute blessure, maladie ou dommage matériel pouvant survenir lors de ma participation aux activités d'arts martiaux. Cette décharge inclut, sans s'y limiter, toute réclamation découlant de la négligence ou de la faute de The Kliq ou de son personnel.
• Je comprends que je suis responsable de mes effets personnels et de toute perte, vol ou dommage pouvant leur être causé pendant mon séjour chez The Kliq.
• Je m'engage à suivre toutes les consignes données par les instructeurs et le personnel de The Kliq en ce qui concerne les consignes de sécurité, les techniques appropriées et la conduite générale lors des séances d'entraînement.
• Je comprends que des photographies ou des vidéos peuvent être prises pendant les activités d'arts martiaux, et j'autorise The Kliq à utiliser ces images à des fins promotionnelles sans rémunération ni autre consentement.
• J'ai lu et compris les termes de cette décharge, et je la signe volontairement, dans l'intention d'être légalement lié(e) par celle-ci.

WAIVER

• I, the undersigned participant, hereby acknowledge and understand the risks involved in participating in martial arts activities at KLIQ MMA INC (referred to as "The Kliq"). I am voluntarily participating in these activities and I am aware that they involve physical contact and the possibility of injury.
• I certify that I am physically fit and have no medical conditions that would prevent me from participating in martial arts activities. If I have any medical conditions, allergies, or injuries, I have provided accurate information above and I will notify The Kliq of any changes in my health status.
• I understand and accept that martial arts training carries inherent risks, including but not limited to strains, sprains, fractures, brain trauma, and other potential injuries. I acknowledge that The Kliq and its instructors will take reasonable precautions to ensure my safety, but accidents may still occur.
• I hereby release and discharge The Kliq , its owners, instructors, employees, and volunteers from any liability, claims, demands, or causes of action arising out of or related to any injury, illness, or property damage that may occur during my participation in martial arts activities. This release includes, but is not limited to, any claims arising from the negligence or fault of The Kliq or its staff.
• I understand that I am responsible for my personal belongings and any loss, theft, or damage that may occur to them while at The Kliq .
• I agree to follow all instructions given by the instructors and staff of The Kliq regarding safety guidelines, proper techniques, and general conduct during training sessions.
• I understand that photographs or videos may be taken during the martial arts activities, and I grant permission for The Kliq to use these images for promotional purposes without compensation or further consent.
• I have read and understood the terms of this waiver, and I voluntarily sign it, intending to be legally bound hereby.
